PRIMARY SCHOOL NURSING PROGRAM
The Victorian Primary School Nursing Program Victorian School Nursing Program: Advice | education.vic.gov.au, is a free health assessment service offered by the Department of Education to all children during their first year of primary school and for some children enrolled in years 1 to 6 upon referral.
POSTCARDS AND ONLINE QUESTIONNAIRE
The Primary School Nursing Program has transitioned from paper-based SEHQ’s to an online format. The families now receive a postcard in an A5 envelope, with information on the Visiting Primary School Nurse program and a QR code to scan to access the questionnaire. School Entrant Health Questionnaire (SEHQ) postcards will be provided for the school for distribution to families this week. A limited number of paper SEHQ will be available if required.
