Dental Van Visit 

Free for children 

The Dental Van will be visiting DSPS in Week 9 Term 4. 

Parents can opt in their children to visit the dentist by completing and returning a The Dental Van visit consent form which will be sent home on today ( Friday 31 October ) 

Timeframes: 

Consent forms will go home in 

Week 4  - Friday 31st October  

Returned by

 Week 7 – Friday 21 November 

Dental Van Visit :

Week 9. Monday 1st Dec, Tuesday 2nd December and Wednesday 3rd December (if required)
