PATH Morning Tea

Our recent PATH Morning Tea provided a wonderful opportunity for families to connect, learn, and hear from guest speakers about the supports available to young people and their families as they transition from paediatric to adult health services.
A special thank you to our guest speakers, Laura O'Connor (Nurse Consultant, Adolescent Transition Service), Isabella (Registrar, Women's and Children's Health Network), and Mathew McCurry (Principal, Hospital School). Together, they shared information about the services in place to support young people and their families through the transition from children's health services to adult health services, helping to ensure continuity of care.
We extend our sincere thanks to Laura, Isabella, and Mathew for sharing their expertise and for the important work they do in supporting young people and families during what is a significant life transition. We also thank the families who attended and contributed to the discussion.
These conversations help strengthen our community's understanding of the services available and ensure that families are well supported as students move towards transition and adult health care services.
