Medical Conditions

If your child has been diagnosed with Anaphylaxis and/or another medical condition which requires him or her to have medication available to them at school it is important that you fill in a medical form and return it to the school office as soon as possible.

 

As per Ministerial Order 706 we have strict rules to follow in order to keep your children safe at school. It is now a requirement of the school that the forms are signed and all medication is current prior to any school engagement. If your child also has a diagnosis of Asthma or Allergies, please also provide the items under these headings. You can obtain all these forms through your local GP and fill them in together.

Anaphylaxis 

1. Red "ASCIA form" -  This needs to be signed by your local doctor (or Specialist) prior to or on  his first day back at school. It also must have a current photo of your child attached to the form prior to it being signed. 

 

2. "Individual Student Anaphylaxis Management Plan 2024" form -  This form is double sided and needs to be signed by the parents of the student. 

 

3. Epipen -  This Epipen must be supplied in its original packaging with a prescription label identifying the correct Epipen for the student. 

Asthma 

1.'Asthma Action Plan" form -  This needs to be signed by your local doctor (or Specialist) and must have a current photo attached to the form. 

 

2."Asthma Management Plan 2024" - This form can be signed by the parents. Salbutamol (Ventolin) and Spacer. 

Allergies and other Medical conditions 

1."Medication Authority Form" -  This form can be signed by the parents and gives us the authority to administer medication as ordered by your doctor. The medication and required dose must be clearly stated. Medication (eg Antihistamine "Zyrtec") must be in the original packaging with your child's name and year level clearly marked on the outside of the box.
