Year 10 Immunisations

As part of the Secondary School Immunisation Program Year 10 Students will have the opportunity to receive their Meningococcal Conjugate ACWY vaccine on the 11th of September. 

Please follow the consent link below and complete the online consent form. 

The consent link has an option to select ‘Yes’ or ‘NO’ to the vaccine. It is important for all parents/guardians to complete the consent link whether it is ‘Yes’ or ‘No’. 

Consent form must be completed by the 5th of June.
