Immunisation Catch-Up Day

Mitchell Shire Council have organised a immunisations catch-up day for all secondary students at Seymour College on Wednesday 11th October 2023. 

This is a once off service for all secondary families, that may have missed an immunisation or two.

Please fill out the attached consent form before the end of term 3, giving consent for the Mitchell Shire immunisation team to look up your child’s immunisation history.

Please send all consent forms to:

immunisation@mitchellshire.vic.gov.au or send a photo of the form to 0407 892 925.

If your child has missed any vaccinations, the Mitchell Shire immunisation team will contact you directly and collect any consent needed prior to the immunisation catch-up day on the 11th October.

Immunisation History Request 

Smile Squad is a Victorian Government program that offers free dental care to all Victorian government school students in all year levels.

The friendly Smile Squad dental team will be visiting Seymour College between the 

16th October – 3rd November 2023

Providing a free annual dental health examination and any follow-up treatment as needed.

Consent forms are being sent home this week, please fill them out and return it to school as soon as you can. 

There are extra copies of the consent forms in the General Office if needed or call and speak with Brooke our school nurse.
