YEAR 10 IMMUNISATIONS

Dear Parent/Guardian, 

On Tuesday 18th March 2025, Barwon Health immunisation service, will visit our Year 10 students to administer the free Meningococcal ACWY vaccine.

It is essential to complete the consent form regardless of whether you provide consent (YES) or decline (NO). Barwon Health Immunisation requires confirmation that you have received the information and made an informed decision for your child. If the consent form is completed, no further follow-up will be necessary.
