LSE Request Form

NEW! - Learning Support & Enrichment (LSE) Request form for Year 10, 11 and 12 students – who require disability provisions or adjustments for in-class assessment tasks:

Students with approved disability provisions or adjustments (DPA’s) in Years 10, 11 and 12 are now asked to complete the following SPX google form, so that requests can be received by LSE Staff, and arrangements can be made to inform teachers, and supervise and provide these students with the DPA’s (Disability provisions or adjustments) that they are entitled to for their timed written in-class assessment tasks https://goo.gl/forms/sRmKGSE8brptjDw73

Parents are welcome to assist their sons in the senior secondary years to complete the above form as needed. You are also encouraged to bookmark and save this link, which can also be found on your son’s SPX portal page

Please note that this form is not needed for exams that are held during major exam blocks throughout the year, namely: the Semester 1 and end of Year 11 Preliminary exams, the Year 10 Semester 1 and end of year exams, the end of year exam block for Years 5 - 9, NAPLAN tests, Year 12 Trial exams, or HSC examinations.

If you wish to discuss a change to your son’s approved DPA’s or if you think your son may be eligible because of the negative impact that a diagnosed disability has on his ability to show his true ability in tests, exams and assessment tasks, then please contact Ms Raheb-Mol on JRahebMol@stpiusx.nsw.edu.au
