SCHOOL DENTAL INITIATIVE

The Dental Health Van will be visiting the college on Tuesday July 12. Consent forms have been handed to students via homerooms. Spare forms are available at the student office at the Enright and Mercy campus if needed.

The School Dental Initiative provides eligible students from all year levels: a full complete dental examination, diagnostic x-ray, remineralisation of teeth and fissure seals if required. This service is bulk billed by the Medicare Child Dental Benefits Schedule. Therefore no out of pocket costs are associated.

If you wish for your child to participate in the initiative, please complete the consent form fully, and return it to the homeroom teacher or student office by Friday May 6.

Please note that this is an "opt-in" service.  All forms must be in physical form, not via email.

If you have any queries, please email me at firstaid@sjcmda.vic.edu.au.
