Support Teacher Transition

 

Hello and welcome to all new families in 2022.  

This is just an introduction to fill you in on my role as the Support Teacher Transition for the Orange/ Lachlan region.

 

My focus is on high school students with disability and additional learning needs, in support classrooms, schools for specific purposes, and in mainstream settings. I am based at Anson Street School, but I also support 12 schools between Blayney and Condobolin.   

 

My role as Support Teacher Transition is to work with school staff and families to assist students to transition through and from school. A particular emphasis is placed on the senior years of schooling. Similar to a Personalised Learning Plan, I carry out Individual Transition Plans (ITP) with students, school staff and families to assist with the transition process.

 

Planning for post-school destinations may take into account the student's:

  • Further education and training  
  • Employment options
  • Transport and mobility requirements
  • Leisure and recreation interests
  • Personal care and independent living skills
  • Future accommodation options
  • Subject selections and pattern of study to support post-school goals
  • Access to specialist programs and support provided by other government agencies and schemes such as the NDIS.

If needed, I will also refer families to outside agencies and supports such as:

  • TAFE disability consultants and counsellors
  • Employment services
  • Organisations that support NDIS clients
  • Centrelink and
  • Advocacy services
