If you need to submit any forms or information to the school please do so via email:  south.melbourne.park.ps@education.vic.gov.au. If this is not an option, call to arrange a time when you can drop them in to the office - T: 9498 3396.

Student medical condition requirements

Dear Parents,

If you have advised us that your child has a medical condition then we require a current medical / action plan authorised by your GP which addresses this condition, as well any specified medication.

If you child has Asthma they must have their medication in their bags when they attend school.

Please note that the school does not have any non-prescribed medicines on the premises, nor are we able to administer any medicine without authorisation by parent/s and a healthcare professional.

It is important that we have current records and medication to ensure that we are knowledgeable, prepared and able to provide the best care to students and a safe and happy learning environment.

2021 Enrolment

We are now taking Enrolments for 2021. Forms can be downloaded from our web site, or contact the school and we can email the form to you. Completed forms should be submitted by the end of Term 2 - Friday 26 June via email south.melbourne.park.ps@education.vic.gov.au

Camps, Sports and Excursion Fund | CSEF

The CSEF program for 2020 classes closes on 26 June 2020. All forms must be submitted to the office by that date. A copy of your current healthcare card is required also.

Immunisation History Certificate

It is a legal requirement to provide a copy of your child's Immunisation History Certificate on enrolment to primary school in Victoria. If you have not provided this information please do so as soon as possible.

In the event of an infectious disease outbreak, unimmunised children and children whose immunisation status is unknown because a certificate has not been presented may be excluded from attending school until the risk of infection has passed as per Department of Education guidelines. We are in the process of contacting families who have NOT submitted the required immunisation statement.
