EDUCATION DEPARTMENT REQUIREMENTS FOR EXCURSIONS 

New Requirements from the Education Department for School Excursions

All Asthmatic Students attending any excursions or camps are now required by the Education Department to have an Asthma Update Form filled out prior to

 attending the event. 

This form is different to the Students' Asthma Action Plan. 

This new form must be filled out by the PARENTS and NOT a doctor. 

The purpose of this form is to inform staff on the excursion of any illnesses (hayfever / colds/ change of medications / recent attacks) that have occurred in the two weeks prior to the event that may trigger the student’s asthma. 

These forms are to be returned THE DAY BEFORE the event so Amanda can prepare the paperwork for the start of the event for the staff.

Please keep an eye out for these forms before any excursion or camp either via hard copy from your child or via email from Amanda.
