Forms for Completion and Return
Please return the below forms to the office by this Friday 13th March. These are a Department of Education requirement to ensure the school has up to date medical and contact information for all students.

Confidential Student Medical Forms
The pink form is updated each year for the purpose of a teacher excursion register and to note if any changes have been made to your child's emergency and medical details. One completed form is required for each student.
Camps, Sports & Excursions Fund (CSEF)
The CSEF will be provided by the Victorian Government to assist eligible families to cover the costs of school trips, camps and sporting activities. If you hold a valid means-tested concession card you may be eligible for CSEF. Contact the school office to obtain an application form. For further information please see the attached flier or visit:

Book Club
Scholastic brochures were sent home last week. The deadline for orders via LOOP is by Thursday 12th March.
How to order and pay
LOOP is the NEW Scholastic Book Clubs Linked Online Ordering and Payment platform for parents. The school does not accept cash for Scholastic Book orders.
Log-in to https://mybookclubs.scholastic.com.au/Parent/Login.aspx
Click on ORDER in the top menu or REGISTER first to save your details for the first time. Select Teesdale PS and your child’s class. Add your child’s name and last initial. Enter the item number from the brochure.
You can order for multiple children at once. All orders are linked directly to the school. Books will be distributed through your child’s classroom.
If parents wish to collect them personally from the office, please let the school know.
MVA Athletics - Years 3-6
When: Monday 16th March
Cost: $14 (Non-refundable)
Where: Goldsworthy Reserve, Corio
Time: Bus will be leaving Teesdale Primary School at 8.15am sharp. Students will need to be at school by 8.00am to begin loading.
A letter and invoice was sent home last week to all families. If you would like to pay by Bpay, please refer to the invoice with your unique biller details. Please remember to return the signed permission envelope and record the payment details for reconciliation by Thursday 12th March.
Young Leaders Conference - Year 6
When: Friday 20th March
Cost: $50 (Non-refundable)
Where: Melbourne Convention Centre
Transport: Students are required to be South Geelong Rail Station no later than 7.00am to catch the 7.12am train.
Parents are encouraged to arrange shared travel for their children to and from the South Geelong Station. We will return on the 3.30pm train arriving back to South Geelong at approximately 4.30pm.

Absences
Parents are reminded that an absence notification is required for all student absences by 11am on the day. The most efficient way to advise an absence is via the Sentral Parent Portal where you can send an email directly to your child's teacher.
Alternatively, please phone the school or complete an absence note available at the office.
Using Sentral / Parent Portal
If you are logging into Sentral for the first time, please do so using a desk top or lap top. A mobile device can be used to log in only after the initial log in has occurred via the above method.

How to message your child's teacher
Log in via the Teesdale Primary School website and click on the SENTRAL Parent Portal tab.
Once you have logged in you should see the Dashboard screen as follows. Click on the Messages tab. (To notify of an absence, do not use the Absences tab. This tab is only used if you are requested to explain an absence).
On the next screen below, on the far right side of the screen, click on New Message.
The next screen below allows you to select your child's teacher and write your message.
Click Send Message, then your message will be sent directly to your child's teacher.
Oxygen Music Lessons
Our school is pleased to offer private music tuition to students from grades 1 to 6 in conjunction with Oxygen Music. Their professional staff are experienced with beginners through to advanced and offer a half hour lesson here at school on Tuesdays during school terms.
