Dear Parents/Carers

I would like to remind parents that it is very important to keep the school updated with your child’s medical history and any new health issues.

This will help us keep your children healthy and safe in school.

Any changes or new health issues need to be updated and recorded on the school system.

If your child needs to take medication at school, please fill out a Medication Authority form and drop it off at the office with the medication.

All Asthma, Anaphylaxis, Epilepsy, Allergic Reactions, Diabetes Medical Action Plans need to be updated yearly by your GP.

A copy of the Action Plan needs to be handed to the office to update medical information.

Please find attached important medical forms that you may need.

Reminder that the Year 7 Vaccinations will be on Tuesday, 17th October 2023.

Please send back consent cards to the office signed and dated whether your child will be having the vaccine or not.

The Whittlesea Council will be collecting the consent cards.

Your child was given the Vaccination consent card at the start of the year in February to take home and fill out.

If you do not have one, please ask your child to come past the office to collect one.
