2021 Health Action Plans, Care Plans and Management Plans

Dear Parents/Carers,

If your child has a General Medical, Allergy, Anaphylaxis and Asthma Plan in place, it is now time to update. 

We need these medical plans to be up-to-date to ensure we can best support the health of your child. Additionally, since 2020, we envisage that it is possible that some students may have been diagnosed with a health condition such as seasonal hay-fever, an allergy, asthma, etc. 

If this applies to a child in your family, please visit this link below, download the relevant planning template, complete this and return it to the College before Friday 15th October, 2021.

Each submitted Plan must be completed by your medical health practitioner.  This includes it being signed and stamped which is a requirement of the Department of Education and Training.
