Medication Authority Forms

Dear Parents/Carers,

 

As part of the Medical Management Policy governing all Melbourne Catholic Primary schools, we are now required to have a Medication Authority Form for all children who are receiving medication onsite (administered by school staff). The form must be completed in consultation with the child’s registered medical/health practitioner and the parents/guardians of the child.

 

The Medical Authority Form needs to be completed for all medications, including long-term medication, short courses eg antibiotics, and occasional medication eg. antihistamines for hayfever or panadol for headaches. As a parent/guardian, you are asked to complete the attached Medication Authority Form, and return it to school at your earliest convenience. This form will require a signature from your child’s medical/health practitioner.

 

For your convenience, we advise you to take a copy of this form to your GP appointment should your child become sick and require a doctor's appointment. If your child is prescribed antibiotics, you will be able to ask your GP to sign this form during the appointment.

 

We thank you for your support in ensuring that all the necessary documentation and requirements are being met, in accordance with school policy.
