A message from EVPS Sick Bay

Welcome back to all families at EVPS! We continue to organise all our students' health care needs for the year. Late last year your child if they’re known to have a health alert was sent home relevant medical/action plans to be completed by their medical practitioner. This is a timely reminder to return them completed to the office as soon as possible.
If your child has Asthma, even if its seasonal, an Asthma action plan must be provided. It needs to be completed and signed by your doctor. A spacer and Ventolin must also be supplied to school. These will be placed into a kit clearly labelled with your child’s name and kept within the sickbay, ready for use as your child needs it.
If your child suffers from Anaphylaxis (RED ASCIA PLAN) or an allergy, (Green ASCIA PLAN) ALL forms must be completed and signed by your doctor. Please also supply the medication listed on the plan clearly labelled in original packaging with your child’s name.
If your child requires medication to be given at school, this must be provided to the main office/sickbay, with a signed medication authority form completed by the parent. Medication must be clearly labelled with your child’s name and grade, be in original packing and prescription as required.
If your child finished at EVPS at the end of 2024, can you please come and pick up any medication/Asthma kits you have supplied from the sick bay/school office. Any medications not picked up by end of week 3 term 1 2025 will be disposed of.
Please return all completed forms to the Main office at the beginning of the 2025 school year.
