School Immunisation Program

As part of the WA School Based Immunisation Program, Year 7 students will have vaccinations in Week 8 this term, on Tuesday, 4 June

Consent (online or paper form) for your child to have their vaccination at school must be completed no later than Wednesday, 29 May (next week)

Please note the following information from the Department of Health, including the necessary steps to provide consent for your child to be vaccinated in the school-based immunisation program. 

  • Immunisation protects your child against vaccine-preventable diseases.
  • Parents, guardians and adolescents can consent for vaccinations via VaccinateWA 
  • If your child misses their vaccine at school, visit your immunisation provider (the vaccine is free, but consultation fees may apply).
  • From January 1, 2024 some Community Pharmacists may offer adolescent vaccines at no cost. Please check with your pharmacy. 

Year 7 students are offered:

Human papillomavirus (HPV) vaccine: 1 doseThe HPV vaccine protects against the human papillomavirus (HPV).Diphtheria-Tetanus-Pertussis (dTpa) vaccine: 1 doseThe dTpa vaccine protects against diphtheria, tetanus and whooping cough (pertussis).Please refer to the attached fact sheets for further information about this vaccine and the immunisation program.

Consent:

Consent must be provided by a parent or guardian for their child to receive vaccinations at school. You are encouraged to provide consent online via VaccinateWA

If you’re not able to access the online system, you can print a copy of the school-based immunisation program consent form. 

These forms are also available online at insert link and copies of the form can also be collected from Reception. 

 Consent must be completed by Wednesday, 29 May.
