Mitchell Shire Council are providing the opportunity for parents/guardians to complete an online digital consent form for Year 7 immunisations.
(if you have already returned a completed hard copy Consent Form to school, this will not be required).
If you have not returned a completed Consent form, advising either yes or no. Please complete the online registration, so the immunisation team are aware of your instructions regarding vaccinating your child for the following immunisations:
Year 7: One dose of Human Papillomavirus (HPV) - Gardasil 9 vaccine and
One dose of Diphtheria-tetanus-pertussis (ie whooping cough) (DTPa) -Boostrix vaccine
The Mitchell Shire Immunisation team will be attending Seymour College on Monday the 6th May 2024.
If your child is absent on this date, they will re-attempt to complete any consented vaccination/s on the 20th May 2024 when they return to our school for the Year 10 immunisation day.

You will need to register your mobile, and then follow the prompts to advise either yes or no to the vaccinations, remember to confirm all the details at the end of the page to complete the record.
