Year 7 immunisation program

Please be advised that the following vaccines will be provided to Year 7 students on Friday 7 March:

Human papillomavirus (HPV) – one dose
Diphtheria-Tetanus-Pertussis (Whooping Cough) – one dose

The Department of Health has introduced a new online consent process to replace paper consent forms. Please use the below link to create a digital profile and provide consent. This must be completed, even if you choose not to vaccinate your child.

We kindly ask that the online consent process be completed by Monday 3 March regardless of your vaccination choice.
