Consent and Combined Permission Forms 2025

On Xuno, there are two forms that require completion please. The first form is a local consent form which includes leaving the school grounds for cross country, community oval etc.

The second form is a combined permission form which includes several questions about movies, head lice, photographs and videos. Please complete these forms for each child as soon as possible.

Both requests have an approval and a form to complete, which involves answering additional questions. Please ensure you complete both.
