The National Assistance Card
The National Assistance Card is a personalised card to assist people with disability and health conditions in the community. This card can be used in everyday or emergency situations where a cardholder needs assistance or support.
All cards are personalised and contain:
First and last name
A photo
Date of birth
Nominated contact person's name and phone number
The National Assistance card can also contain:
Up to 5 areas of difficulty chosen by the cardholder (e.g. communication, fatigue, memory, processing information)
A QR code providing access to additional written information or a personalised video
The police assistance line phone number
Cardholders have the option of adding additional information to their QR code about the impacts of their disability and how people in the community can assist them.
The National Assistance Card provides cardholders with a means to communicate the impacts of their disability and any assistance they may require. It is also a way to raise awareness of disabilities in the community as well as provide reassurance to families and carers knowing a cardholder can be assisted in their absence. This card will also help community members including first responders respond appropriately when shown the card.
