First Aid Requirements

This is a reminder that all medication that is sent to school must have a Medication Administer Form fill in by your Doctor for us to be able to administer. Please do not send anything into the office unless you have the form.

I have attached the form and I have also sent it out on Seesaw so you have access to it at all times.

If your child's circumstances changes over the holidays please make sure to bring in the new paperwork first day back and inform us at the office.

I will be sending home notification of medication that needs to be replace at the beginning of the year. Please make sure to ask your children for any notes. I will be sending home on Monday.
