Medical Conditions 

We remind all families that as part of your EnrolmentAgreement, you are required to notify St Justin's immediately should your child's medical needs (and/or medications) have changed and that you update nForma with this information.  You are also required to provide original copies of the updated relevant Action/Care Plan, Medical Management Plan and Medication Authority Form (Signed by an AHPRA registerd practitioner) to the office so that we can ensure the appropriate level of care is provided for your child at all times.
