                      IMPORTANT INFORMATION                       MAF-Medical Administer Form

 

Dear Parents,

 

Please read the following information about the procedure in relation to administering medication to your child/ren.

I have attached the relevant form which are required to be completed by your Medial Practitioner.   

This procedure began at the beginning of 2024 which means unless we have this paperwork NO medication can be administered by staff. 

If there is no form you will need to come in and administer the medication yourself.   

Once we have the form we are obliged to only administer medication listed on the form. We are not allowed to give the medication at any other time or another dosage other than what is on the form.

Whenever you go to the Doctor make sure to have the form with you in case they prescribe medication in which case get them to fill in the MAF also. It would be a good idea to scan it to yourself so you can access it from your phone at any time.                                                               This is what we have received from our head office.                                                                                                          

  1. Parents / guardians / carers are responsible for providing a Medication Authority Form (MAF) to the school via a hard copy form that has been sent home containing the medication/s to be administered, and the medication requirements (dosage, method of administration, frequency, times of day, storage) at the time of enrolment or when notified of the required administration of medication in the school environment.
  2. Medication must not be administered to a child being educated and cared for unless the MAF is authorised by an Australian Health Practitioner Regulation Agency (AHPRA) registered medical / health practitioner except in exceptional circumstances.
  3. The principal or delegate may agree to proceed with a MAF signed by only the parent/ guardian/carer in exceptional circumstances, for the administration of over-the-counter medication at school or during off-site activities such as camps.
  4. The principal or delegate is responsible for ensuring that a MAF is received and updated when the medical advice regarding the medication method, timing, and dosage changes.
  5. Parents / guardians / carers are required to provide the school with the medication in its original packaging, within the expiry, with the name of the student (for prescription medication) and outlines instructions for administration/application of the medication. 
  6. No over the counter or prescribed medication is to be administered beyond the instructions on the original packaging unless written recommendation is provided by an AHPRA Registered Medical Practitioner.

St Anne's Playgroup

 

Playgroup runs every Monday morning in the hall from 9-11am. Please bring a piece of fruit and a $2 donation. It is a great way to meet new parents, while giving the little ones a chance to have a play. No need to book just come along.

The School Family Fee will be payable in three instalments of ($702, $702 & $701) over the first three terms of the school year. The student Levy for all students for the 2025 school year will be $585. To provide better transparency as to how the school finances and income received by parents is utilised, we have again this year broken down the Student Levy to identify what contributes to the levy and how it is spent. In 2025 there will be a Swimming Levy of $60.00 for every year 3-6 student (Foundation – 2 will not be having swimming lessons). This levy is required to help offset the considerable cost to the school. It is a government requirement that all students can swim 50 metres by the end of their Grade 6 year.  The Grade 5 & 6 School Camp will be held in 2025 with a cost of the camp being $380.00. The camp fee must be paid no later than one week prior to the date of the camp. Families who are currently in arrears for the 2024 school year and have not made an arrangement with the school, we ask that you attend to your account as soon as possible or to contact the office. Direct EFT payment details are : BSB 083-347 Account Number 644898266 (Please use family name as reference) Families in arears will be asked to enter into a payment plan to ensure the arrears do not become unmanageable. Please note that we do everything we can to hold costs down whilst providing the best education opportunities possible for your child. Statements, Direct Debit forms and information will be sent out at the beginning of Term 1 2025 in hard copy. To start a payment plan for 2025 please complete the Direct Debit form and return to the School Office. School Fee Concession 2025With the change in parish school governance to MACS (Melbourne Archdiocese of Catholic Schools) there has been a change to assist school families experiencing financial hardship in accessing a Catholic primary education for their child/children. There is now the opportunity to apply for a School Fee Concession in 2025. At all times, the dignity and privacy of those seeking a fee concession will be respected. School office staff can advise applicants of the concessional fee to be charged. This concessional fee is fixed annually by Melbourne Archdiocese Catholic Schools (MACS). Approved applicants will be charged only the annual concessional fee, which will cover all standard school fees and levies. If the applicant has become eligible under one of the eligibility criteria listed below throughout the year, a pro-rata concession will be granted from the time the application is made. In this instance, standard school fees and levies will be incurred for the portion of the year whereby the applicant was not eligible. To be eligible for the concession, the family must agree to a fortnightly direct debit. Application for School fee Concessions will be available to families under the following categories: Aboriginal or Torres Strait Islander heritageServices Australia Health Care Card holderDepartment of Veterans’ Affairs Gold Card holderDepartment of Home Affairs ImmiCard (proof of identity card) holderRefugee, where the child has attended a school in Australia for less than five years  Please do not hesitate to contact the Office for further information in regard to the concessional fee application.  May I also take this opportunity to thank the many parents who have supported the school in a non-financial way through their participation in classroom activities and parent groups such as the School Advisory Board and P &F Committee. Advent Sunday is what many Christians call the first day of Advent. The theme of the first week of Advent is \u003cstrong>hope\u003c/strong>. I don't know about you but I certainly have a hopeful feeling. Hope for a joyful end of the year for the students, hope they don't have too many nerves about their coming change of class or school, hope that I find time for stillness in this busy and fun time as we enter into Summer. Time for reflection, appreciation, gratefulness and the promise that Jesus' birth brings. Formally this theme emphasizes the hope Christians have in the coming of Christ, reminiscent of the anticipation of the Messiah in the Old Testament. We are having an Advent Liturgy each week as we develop with the students their understanding of preparation and waiting. \u003c/p>\u003cp> \u003c/p>\u003cp>Our Market Day has been and gone and, despite the weather, was a big success. Michele has reported that we earnt around $8250 which given the weather conditions is really an amazing achievement and one the organising committee, and parent body as a whole, should be proud of. Thank you. \u003c/p>\u003cp>We are still not entirely sure of what was raised from the Lapathon for the Christmas Hampers to be supplied to families in need by our local St Vincent de Paul's chapter.  I will let you know hopefully in next week's newsletter. \u003c/p>\u003cp> \u003c/p>\u003cp>Last Friday our students had a chance to hear their peers who take part in \"Kids on Key\" perform. Our young musicians had obviously worked hard this year and their parents, staff and peers must have been very impressed. Though this is an 'extra' it is easy to see that it is something that many students have enjoyed doing and has improved their skills whether as a guitarist, keyboardist, ukelele player or singer. To have an audience made up of  your peers must have been a little bit daunting. I thought they were great and didn't want it to end. \u003c/p>\u003cp> \u003c/p>\u003cp>Our Foundation 2025 students have now had three Orientation sessions and the students must be very confident about beginning their schooling at St Anne's in February next year. They have their Teddy Bear's picnic on Friday 29th between 2-3pm and their final Orientation session on Tuesday 3rd December. \u003c/p>\u003cp style=\"text-align:center;\"> \u003c/p>\u003cp>Today we have had Frankston City Council Youth Workers meeting with our Year 6 students about their transition to secondary schooling. We hope this has helped them to manage the transition even more successfully as their time at St Anne's draws rapidly to a close. \u003c/p>\u003cp> \u003c/p>\u003cp>Next week we have the classrooms open Monday morning for a while as Mr Sullivan has shared via seesaw. We like to celebrate our students as they have worked very hard this year with some great results with our testing showing so much growth for so many. Wednesday night is our Christmas Concert at 6pm. Our classes are all singing and all dancing at the moment in preparation and it looks like a lot of fun! \u003c/p>\u003cp> \u003c/p>\u003cp>In preparation for next year I am finalising staffing and I will be in a position to share this with you all in next week's newsletter. In talking with other Principals the market is still very tough out there for finding quality teachers so I am very grateful for the great team we have assembled at St Anne's. Mrs Tracey Di Paolo is retiring after being on leave this year. She has had a long and distinguished career in Catholic Education and her passion for Literacy in particular has been second to none. It has been a pleasure to have her working with many of your children over the past decade. We wish her and her husband Emilio a fantastic retirement. Advent Sunday is what many Christians call the first day of Advent. The theme of the first week of Advent is hope. I don't know about you but I certainly have a hopeful feeling. Hope for a joyful end of the year for the students, hope they don't have too many nerves about their coming change of class or school, hope that I find time for stillness in this busy and fun time as we enter into Summer. Time for reflection, appreciation, gratefulness and the promise that Jesus' birth brings. Formally this theme emphasizes the hope Christians have in the coming of Christ, reminiscent of the anticipation of the Messiah in the Old Testament. We are having an Advent Liturgy each week as we develop with the students their understanding of preparation and waiting.  Our Market Day has been and gone and, despite the weather, was a big success. Michele has reported that we earnt around $8250 which given the weather conditions is really an amazing achievement and one the organising committee, and parent body as a whole, should be proud of. Thank you. We are still not entirely sure of what was raised from the Lapathon for the Christmas Hampers to be supplied to families in need by our local St Vincent de Paul's chapter.  I will let you know hopefully in next week's newsletter.  Last Friday our students had a chance to hear their peers who take part in \"Kids on Key\" perform. Our young musicians had obviously worked hard this year and their parents, staff and peers must have been very impressed. Though this is an 'extra' it is easy to see that it is something that many students have enjoyed doing and has improved their skills whether as a guitarist, keyboardist, ukelele player or singer. To have an audience made up of  your peers must have been a little bit daunting. I thought they were great and didn't want it to end.  Our Foundation 2025 students have now had three Orientation sessions and the students must be very confident about beginning their schooling at St Anne's in February next year. They have their Teddy Bear's picnic on Friday 29th between 2-3pm and their final Orientation session on Tuesday 3rd December.  Today we have had Frankston City Council Youth Workers meeting with our Year 6 students about their transition to secondary schooling. We hope this has helped them to manage the transition even more successfully as their time at St Anne's draws rapidly to a close.  Next week we have the classrooms open Monday morning for a while as Mr Sullivan has shared via seesaw. We like to celebrate our students as they have worked very hard this year with some great results with our testing showing so much growth for so many. Wednesday night is our Christmas Concert at 6pm. Our classes are all singing and all dancing at the moment in preparation and it looks like a lot of fun!  In preparation for next year I am finalising staffing and I will be in a position to share this with you all in next week's newsletter. In talking with other Principals the market is still very tough out there for finding quality teachers so I am very grateful for the great team we have assembled at St Anne's. I can however let you know who will be leaving us at the end of the year. As you may be aware Mrs Jemima Holmes will be taking maternity leave, which we wish her all the best. Mrs Tracey Di Paolo is retiring after being on leave this year. She has had a long and distinguished career in Catholic Education and her passion for Literacy in particular has been second to none. It has been a pleasure to have her working with many of your children over the past decade. We wish her and her husband Emilio a fantastic retirement. Our final person moving on is Mrs Lauren Lacey who began her teaching career here at St Anne's about 6 years ago. She will continue the next stage of her teaching career at Botanic Ridge PS and I'm sure that they will be as excited, as we are sad, to have her join their team. Congratulations!School Student Broadband Initiative                       IMPORTANT INFORMATION                       MAF-Medical Administer Form Dear Parents, Please read the following information about the procedure in relation to administering medication to your child/ren.I have attached the relevant form which are required to be completed by your Medial Practitioner.   This procedure began at the beginning of 2024 which means unless we have this paperwork NO medication can be administered by staff. If there is no form you will need to come in and administer the medication yourself.   Once we have the form we are obliged to only administer medication listed on the form. We are not allowed to give the medication at any other time or another dosage other than what is on the form.Whenever you go to the Doctor make sure to have the form with you in case they prescribe medication in which case get them to fill in the MAF also. It would be a good idea to scan it to yourself so you can access it from your phone at any time.                                                               This is what we have received from our head office.                                                                                                          Parents / guardians / carers are responsible for providing a Medication Authority Form (MAF) to the school via a hard copy form that has been sent home containing the medication/s to be administered, and the medication requirements (dosage, method of administration, frequency, times of day, storage) at the time of enrolment or when notified of the required administration of medication in the school environment.Medication must not be administered to a child being educated and cared for unless the MAF is authorised by an Australian Health Practitioner Regulation Agency (AHPRA) registered medical / health practitioner except in exceptional circumstances.The principal or delegate may agree to proceed with a MAF signed by only the parent/ guardian/carer in exceptional circumstances, for the administration of over-the-counter medication at school or during off-site activities such as camps.The principal or delegate is responsible for ensuring that a MAF is received and updated when the medical advice regarding the medication method, timing, and dosage changes.Parents / guardians / carers are required to provide the school with the medication in its original packaging, within the expiry, with the name of the student (for prescription medication) and outlines instructions for administration/application of the medication. No over the counter or prescribed medication is to be administered beyond the instructions on the original packaging unless written recommendation is provided by an AHPRA Registered Medical Practitioner.  Thank you for your assistance in this matter.
