Onsite Attendance Form 

Parents and carers who require their child to attend on-site school learning will need to complete an On-site School Attendance application form in the week prior to their child attending on-site. 

 A new form must be completed on a weekly basis (by Friday 12pm) in order that the school can make appropriate staffing arrangements. All Victorian Government schools will continue to offer on-site programs for eligible students (essential workers, vulnerable, students with disabilities).
