Vaccination Offer for Senior Students

We are very pleased to offer our senior students the opportunity to receive a Pfizer vaccination. The Corangamite Clinic has offered the free services of a medical doctor to deliver these vaccinations on site here at Trinity College.
This is an optional program and and the immediate response when we first circulated information to families yesterday was very strong.
There are limited bookings available so please respond asap if you would like your child to be vaccinated here at school.
The offer will be available only for the first dose of the Pfizer vaccination at this stage, although this is dependent on the demand and supply of vaccinations.
Eligible students: available for all Year 12 students as well as Year 10 and 11 students that are writing a Unit 3/4 exam.
Date: Tuesday, 7th September 2021
Venue: Tuck Wing Trinity College
Time: students will be allocated a staggered time schedule which will be confirmed once the administrative details of the application have been received.
The Process:
- Step 1:
Families must complete a form available at the following link to indicate they wish for their child to receive the vaccination https://forms.office.com/Pages/ResponsePage.aspx?id=zFiu6TorLk6Ax8nE_LhQtvxB8w2NrMFAiK1dyjZJzYRUM09TNUdOM1pOSlhMQjdGTEo3NTc0VkI5Qy4u. Complete the required personal details for the student in the form. The student will also be referred to as the 'patient' in correspondence. The option to apply for this vaccination will close on Friday at 10am so that patient numbers can be arranged with Corangamite Clinic.
- Step 2:
There is an additional medical form entitled COVID-19-Vaccine-Patient-Consent-Form which must be completed and brought to the school on the day of the vaccination. An electronic copy of this form is available on the Corangamite Clinic website and was attached to the information sent to families and students. No vaccination can be administered without this completed consent form. However, regular patients at Corangamite Clinic can complete this form through the ‘Hotdoc’ app.
- Step 3:
Parent/Carers must give consent for their child to receive this vaccination via PAM. Please click on PAM for these details.
- Step 4:
Once we have received all the applications, the available vaccination times will be communicated early next week to families.
Note:
- The family or student do not need to be a patient at Corangamite Clinic to accept this offer.
- Corangamite Clinic is volunteering this service so there is no cost.
