2021 OSHC Enrolment Forms 

You can collect an Enrolment Form from either the office or OSHC room.

Please provide updated medical forms and immunisation schedules. 

Additionally, if your child is no longer coming to OSHC, please either call or email to inform us of this, otherwise a fee will be charged.

Reminder to Parents

Parents please check mygov to update your CCS details.

Some children who haven’t attended the program for a while may have had been ceased, and we will have to activate your benefits.

We require all families to complete a new enrolment form for this year 2021.

           Curriculum Day Tuesday 9th March

OSHC will provide Breakfast and afternoon tea and activities.

Parents please supply your child/ren with morning tea, lunch and a drink bottle.

Children don’t have to wear there school uniform.

If children bring items from home it is there responsibility to look after them.

Cost for the day program is $45.00
