Medical information
The Department of Education and Training have updated their medical information policy. Due to the changes within the policy, schools are now required to update student medical plans annually for medical conditions listed below.
If you have a child with any of the following medical conditions, a plan that has been completed by a medial practitioner, i.e. Doctor, must be supplied to the school.
- Anaphylaxis
- Asthma
- Allergy (also medication authority form must be filled if supplying medication to the school)
- Diabetes (also medication authority form must be filled if supplying medication to the school)
- Epilepsy (also medication authority form must be filled if supplying medication to the school)
The Department of Education and Training have strict guidelines regarding the type of plans schools are allowed to accept, the medical plan templates can be collected from the General Office.
If you have not already supplied a plan for your child this year and your child has any of the above conditions a plan MUST be supplied to general office no later Tuesday 12th March 2019.
